Company H
- Captain Patrick McNally – Original captain; recruited the company in Lawrence County, Ohio; mustered in June 28, 1861; promoted to major.
- Captain Joseph M. Bushfield – Succeeded McNally as captain.
- Captain John Combs – Later captain of the company.
- First Lieutenant Henry C. Flesher – Original first lieutenant.
- First Lieutenant Louis P. Salterbach – Later first lieutenant.
- Second Lieutenant Charles H. Day – Original second lieutenant.
- Second Lieutenant John C. French – Later second lieutenant.
- Second Lieutenant Andrew P. Russell – Later second lieutenant.

Unassigned Recruits
Some recruits and veterans remaining in service after June 29, 1864, were transferred to the 6th West Virginia Cavalry on December 14, 1864.
Consolidations or Transfers
Company H was originally organized as part of the 2nd West Virginia Infantry (mounted in June 1863) and redesignated as Company H, 5th West Virginia Cavalry on January 26, 1864. Upon expiration of service, remaining veterans were transferred to the 6th West Virginia Cavalry.
