The regiment consisted of four companies from the 62nd Battalion (Union County), five companies from the 56th Battalion (Morrow County), three companies from the 6th Battalion (Crawford County), and one company from the 89th Battalion (Marion County), all Ohio National Guard units. Upon organization, the regiment departed Camp Chase for Washington, D.C., on the evening of May 13, 1864.

Arriving in Washington, the regiment was assigned to garrison duty at Forts Ellsworth, Williams, and North, which were part of the defensive works south of the Potomac River. The 136th Ohio Infantry remained on garrison duty throughout its service, helping to secure the capital during a period of increased Confederate activity in the region. The regiment’s term of service expired on August 20, 1864, and it was mustered out at Camp Chase on August 30, 1864.
